The tension it names is a measurement problem before it is an ethics problem. Engagement numbers are cheap, immediate, and already instrumented; evidence about long-term effect is slow, expensive, and often unflattering. Left to self-report, an organization will publish the first and describe it as the second.

That is why this principle points toward external assessment. A company scoring its own long-term effect on the people who use it is grading work it has every reason to pass; an outside benchmark is what makes the claim checkable.
